MSA CPD Activities Application Form

  1. This application form has to be filled up before approval of funds for CPD activities is granted. Applications should be made at least a month before the date of the activity.
  2. The maximum amount available is RM4000 per region per year. Additional requests will be considered on a case to case basis.
  3. This funding is only to be used to cover travel and accommodation expenses for the speakers/facilitators. Airfare is for economy class travel only.
  4. After approval by the MSA Exco, the amount of funds requested from MSA will be disbursed within two weeks to the coordinator of the activity.
  5. A statement of accounts and original receipts amounting to not less than the sum requested from the MSA must be submitted within one month of completion of the meeting/workshop to the President of the Malaysian Society of Anaesthesiologists. Failure to do so will jeopardize future applications for funding from the MSA.
  6. You are strongly encouraged charge a discounted fee for MSA members. This will enable the MSA to use these CPD activities to recruit more members.
